The crash involved a truck and multiple cars just after midday on Main South Road at Wattle Flat, south of Adelaide.
Two women, who were rear passengers in one of the cars, both died of their injuries.
A 44-year-old woman died at the scene and the second woman died later in hospital.

The driver of the ute, a 48-year-old from Hay Flat, was taken to hospital in a serious condition.
The driver of the black Ford, a 38-year-old woman and her two passengers, a 45-year-old man and a 10-year-old girl, were all taken to hospital.
However, police say their injuries are not believed to be life-threatening.

The Victor Harbor man was arrested and charged with two counts of cause death by dangerous driving, two counts of cause serious harm by dangerous driving and four counts of cause harm by dangerous driving.

The deaths are the 74th and 75th lives lost on South Australian roads this year.
